Meridian Health Services is currently seeking a full time dayHealth Educator Nurse - LPNto be responsible for the patient health education program for our primary care location inRushville, RushCounty, Indiana. This position works collaboratively with the front and back office clinical staff to provide patients with general health education as well as on health issues identified by the patient's primary care provider. The Health Educator Nurse will be a member of a medical care team in accordance with NCQA - Patient Centered Medical Home standards. Key Responsibilities: Treatment Plan Update Case management services Skills, development and self-management training Takes vital signs Administers injectable medications Dispenses sample medications Gathers health histories Collects information about clients medications, OTC meds ad herbal remedies; including strengths, routes of administration and frequency of use, for purposes of medication reconciliation Monitors Medication side effects (MTS) Monitors medication compliance (MTS) Provides education & training re: medications (MTS) Conducts client education about medical conditions and their care Cleans and monitors medical equipment including monitoring for accuracy Waived testing - urine pregnancy test, urine dip, hemoglobin A1c, strep A, influenza A & B, mono, RSV, glucometers, hemoccult blood, H.pylori, and urinedrug panel Other Functions/Duties: Participate in Chronic Care/Patient Centered Medical Home huddles Participate in Quality/Performance Improvement committee(s) Maintain a library of resources on acute and chronic patient conditions to assist providers in educating patients on health conditions Respond to requests from patients concerning effectiveness and/or problems withprescribed medications including side-effects and special precautions Assesses medical services clients to determine high risk clients and workdirectly with medical provider and care team Does outreach calls for health maintenances issues and chronic diseasemanagement Provides support and coaching for patients/ planned care teams Prepares for, attends and participates in team meetings and huddle(s) Chronic disease care management Medication reconciliation and education Active in patient education, goal setting, self-management teaching and coaching We are seeking candidates with the following qualities: Graduation from an accredited Nursing program Current LPN or RNlicensure in the state of Indiana Two(2) years of previous LPN experience in an outpatient setting preferred Two years health educator experience preferred Compensation based on education, licensureand experience starting at $15.77 per hour.
