Job Description

Provide psychosocial assessments and treatment recommendations, as well as discharge planning to and consultation about patients and families to assist them and the health care team in coping with patient's hospitalization, illness, diagnosis, treatment, and/or life situation, including emotional, mental, and substance abuse disorders. Facilitates transfers to other acute care hospital settings or skilled nursing facilities. Posting Department Case Management Posting Department Description Our Case Management Department uses the Clinical Case Management model including an RN and MSW coverage for all patient areas. This team is responsible for patient throughput, including, but not limited to: discharge planning; community resources utilization for patients; transfers - acute care nationwide and international, nursing home, home, and so forth; working with insurance companies; partnering with nursing staff, rehab, the patient/family and physician for effective and efficient delivery of care utilizing the patient's available resources. Area of Interest Professional Start Date Open Until Filled Work Week 40 hrs Daily Hours 8 hrs Bi-Weekly Hours 80 hrs External Education Education Required: Master's Degree in Social Work.

Education Preferred: Social Work with Health care setting Educational Track. CMSW or CISW Certification by Arizona Department of Health Services External Experience Experience Required: 3 years of Social Work experience. Computer proficiency (must be able to pass computer proficiency assessment).

Experience Preferred: Experience with EPIC Electronic Health Record or similar electronic record systems. Requires Call No Relocation Yes Job Status SALARY-FT
